Everybody Active Everyday Shropshire is a physical activity project which aims to support a small number of Shropshire Care Homes who are willing to critically assess and challenge their current practice, and from this develop and implement improvements to increase mobility in the care home.

The aim is for care homes to develop ways which enable every resident to move more often every day. It may be a small change or require many changes, which includes moving toward a mobility ‘culture’ where each person is supported to remain active.

Selected care homes will have access to an allocation of EAED funding to enable them to develop and implement their chosen improvement/s. The funding is not a grant or award and is a once only fund. This may be up to £5000 per care home

If your care home is selected you will be offered support and guidance throughout your project.

 

The Process:

1.If you were unable to attend the Mobility in Care Homes Launch Event on Monday 17th July 2017 it is recommended you Read the Presentations delivered by the three speakers so that you are clear what the project is about.

The three presentations are all entitled Mobility in Care Homes,

1) Janet Gittins: Locality Commissioning Group Manager North Shropshire

2) Miranda Ashwell: Programme Lead Shropshire Public Health

3) Sue Bell: Falls Prevention Project Lead/Practitioner, Shropshire Partners in Care.

2. Expression of Interest

Expression of Interest (self- nomination). There are a number of questions in this form which enables you to tell us why you want to be involved with the project, and formally registers your interest.

The SUBMISSION DATE for Expression of Interest is 5pm TUESDAY 29thAUGUST 2017

 

If returning your Expression of Interest electronically please send to Sue Bell: sbell@spic.co.uk

 

If posting , it is recommended you send by Royal Mail Special Delivery to arrive by 1pm. Mail delivery to our Annscroft site is variable so to ensure we receive your Expression of interest we recommend this method . Retain proof of posting. You can check on the Royal Mail website to confirm delivery to us. Post to Sue Bell, Shropshire Partners in Care, 6 Farriers Business Centre, Annscroft, Shrewsbury SY5 8AN.

 

Late submissions will not be accepted.

 

3. The Selection Process.

The Selection Panel will meet 4th September 2017 to review all submitted Expressions of Interest and undertake the selection process against a selection criteria.

More than one care home may be selected.

The decision of the selection panel is final. There is no appeals process.

If your care home is selected you will be contacted via the email address provided on your self- nomination form, unless you advise us otherwise.

Only the selected care homes will go on to the next stage in the process
