SPiC has been in discussion with the CCG who are currently leading on a focussed and time specified Audit to review the Quality and Safety of discharges from SaTH.

We know from your feedback that on occasion you do experience concerns relating to safe discharge of individuals to your care from the hospitals and we welcome this chance for our Members to be involved and contribute to this audit.

The CCG are keen  to capture feedback from Care Sector colleagues, and would welcome your input and in particular with regards to the following key areas of potential concern:

  • Fact Finding Assessments (FFA) accuracy and relevance of completion to inform safe continuity of care – noted that the FFA has recently been abbreviated to facilitate more effective and timely discharge for those patients on the Discharge to Assess pathway.
  • Medication  – lack of information/supplies/inaccurate prescribing/cannula left in situ
  • Poor discharge summaries – lack of pertinent information to ensure the safe transfer of care
  • Unsafe discharge – timeliness/inappropriate clothing/no handover to ensure continuity of care/failure to organise specialised equipment/A&E discharge summary not routinely shared with Care provider particularly during out of hours’ attendance compromising safe continuity of care.
  • End of Life – Incomplete Respect documentation/lack of information on discharge summary particularly in relation to medication and authorisation requirements
  • Evidence of Excellence – capturing of effective and well co-ordinated discharges to use on an exemplar basis.

The audit period is from now until 31 December 2020.
